Eye Surgical-Medical Associates accepts most major medical health insurance plans, including Medicare, Independence Blue Cross, Aetna, Blue Shield, United Healthcare, Cigna, Keystone First, Health Partners, and Cigna Healthsprings.
The vision plans we accept are Aetna, EyeMed, Davis Vision, LEHB, National Vision Administrators, Superior Vision, Vision Benefits of America, and Vision Service Plan.
The type of insurance used will depend on the nature of your visit.

If your insurance plan requires a referral from your primary care provider, please obtain one prior to your visit. Our NPI number is 1558305219. Please make sure your primary doctor selects the option for “ophthalmology” when entering a referral.
Most insurance plans require you to pay a co-pay, deductible, and/or co-insurance. All of these amounts are set by your insurance plan. Please come to your visit prepared to pay your co-pay, deductible, or co-insurance. Eye Surgical accepts payment in the form of cash, checks, and credit cards.
Every effort is made to verify your insurance benefits prior to your visit in order to give you an estimate for any out-of-pocket costs (co-pays, deductibles, co-insurances) that you might be responsible for. Please keep in mind we can provide only an estimate based on your insurance company’s benefit information available to us at the time we prepare the estimate. A quote of benefits from your insurance company is not a guarantee of payment. We cannot always determine what services you will require and receive in advance, so we may not be able to provide a complete estimate of your cost prior to treatment.

Charges by our physicians for services performed at either the Liberty Eye Surgical Center or Nazareth Hospital do not include charges for facility fees associates with those locations. Patients will be billed separately by Liberty or Nazareth for facility charges.
